Registrar Forms

  • This form is for students currently attending Western. If you have been accepted, but have not started attending yet, please change your major when you fill out your registration survey.

  • If you don’t know, leave this blank and we will contact you if necessary
  • Please list complete program of study. List exactly how you want to appear in DegreeWorks.
  • Also include information that is not changing.
  • If you’re changing your major and know who you want as your advisor, please list them below.
  • If left blank we will assign you an advisor.
  • (Required if adding a second major)
  • If you are filling out this form with your new advisor, please have this advisor enter the last three numbers of their Employee ID.
  • Changes will be processed and available for review in DegreeWorks as soon as possible. You will be notified by email when your changes have been made. At that time, please verify the information is appearing in DegreeWorks as you expected. Please contact the Office of the Registrar in Taylor 300 or email registrar@western.edu if you find any errors.

    DO NOT RESUBMIT THIS FORM UNLESS YOU ARE MAKING ADDITIONAL CHANGES. If you need to verify that we have received your request, please contact our office at 970-943-7012 or registrar@western.edu.
